- Educational Qualification - MBBS Degree with One Year of Internship. MD/MS in respective discipline.
- Age Limit - Maximum 33 years

- Fill the form in proper manner & paste recent photograph.
- Attach all necessary documents & pay the fees through bank DD.
- Submit the application to official address.
