5. Finally submit the app. form copy attach with attested copy of necessary documents.

- Educational Qualification :- For Post 1 is Diploma in General Nursing. For Post 2 is passed class 10th/SSC with qualify in ANM. For Post 3 is D. Pharm or equivalent. For Post 4 is B.Sc degree with DMLT.
- Age Limit :- Max. 38 years
