2. Take a printout of app. form and fill the all details as required & make a signature with your hand. 
3. Paste their recent passport size photograph in proper place. 
4. Pay the app. form through demand draft in fever of "www.sambalpur.nic.in". 
5. Finally submit the app. form copy attach with attested copy of necessary documents. 
 
Selection Procedure :- The candidate must be selected on the basis of Written Test , Skill Test & Interview.
